Scleral Lens & PROSE
PROSE is a treatment that uses a custom fluid-filled scleral device to vault over a damaged cornea. For some people after SJS/TEN, it is the difference between functional vision and not.
What a scleral lens does
A scleral lens is larger than a normal contact lens. Instead of resting on the cornea, it rests on the white of the eye and arches over the cornea entirely, holding a reservoir of sterile saline against the surface all day. That does two things at once: it gives light a smooth optical surface to pass through, and it keeps the cornea continuously bathed rather than exposed.
Both matter after SJS/TEN. Scarring and an irregular surface distort vision in a way glasses cannot correct, and a damaged tear film leaves the eye persistently dry and painful. A scleral device can address both at once.
PROSE versus a standard scleral lens
PROSE (Prosthetic Replacement of the Ocular Surface Ecosystem) is a specific treatment programme from BostonSight, not just a lens. The device is designed from scratch for one eye, and fitting happens over several days with adjustments in between. It is generally used for the more severe and irregular surfaces, which is why it comes up often in SJS/TEN.
Standard scleral lenses are fitted from a trial set by many more practices and are a reasonable starting point for milder disease.
